Planning for gender-affirming surgery often includes practical questions about insurance, authorization, and cost. At Align Surgical Associates, our team works closely with patients to clarify next steps, answer questions, and help you understand both insurance and out-of-pocket options for paying for your procedure.
Using Insurance at Align Surgical Associates
Our office works with most insurance plans and is highly experienced at helping patients navigate the insurance process. Following your consultation, you will meet with our administration and financial team, who will explain the insurance process and what information we need to move forward.
Please be prepared to provide:
- A copy of the front and back of your insurance card
- Information about your primary insurance plan and any additional insurance policies you have
- Updates about any insurance changes
If you have an HMO plan, you will also need to provide a referral from your primary care provider within 90 days of your surgery date.
Insurance Review and Next Steps
Our team will review your insurance information and guide you through the steps of the authorization process. Depending on the procedure and your medical history, this may include referrals, letters of support, medical clearance, labs, or other supporting documentation.

Insurance Approval for Out-of-State and International Patients
If you are traveling for surgery from outside California or from another country, we recommend confirming your approval and coverage requirements before making an appointment.
For our Canadian patients, including those traveling from Ontario or other provinces, you may need to contact your doctor, local health ministry, or insurance provider to determine your eligibility for out-of-country surgery and to obtain the written approval and necessary supporting documentation to schedule treatment.
Paying Out of Pocket for Gender-Affirming Surgery
If you are planning to pay out of pocket for your surgery, we will provide a personalized quote based on your surgical plan. Costs can vary depending on the procedure involved.
Our financial team will review the payment process with you after your consultation, so you know what to expect before moving forward. We also work with third-party financing providers, such as CareCredit®, to help you pay for your treatment over time.
