Forehead contouring is a gender-affirming facial procedure that reshapes the forehead and brow. Depending on your anatomy and surgical plan, it may be used to reduce brow prominence, reshape the upper forehead, or refine brow contour. People often choose the procedure to create a smoother, more balanced upper-face contour that feels consistent with how they want to be seen.
At Align Surgical Associates in San Francisco and Los Angeles, California, forehead contouring is planned with close attention to the underlying bone structure, the brow area, and the overall upper-face contour.

What Is Forehead Contouring?
Forehead contouring is a surgical procedure that reshapes the bones of the forehead and brow region. Patients may also hear it described as brow bone reduction or forehead recontouring, depending on the specific changes being discussed.
In some cases, the brow and forehead can be reshaped with more limited contouring. In others, the anatomy of the brow bone and frontal sinus calls for a more involved approach, such as repositioning the bone to achieve a smoother contour.

Who Might Consider Forehead Contouring?
Forehead contouring may be a meaningful option for transgender individuals, nonbinary patients, and other gender-diverse people seeking changes in the upper face.
This procedure is often used in facial feminizing planning to reduce brow prominence or change the forehead contour. In selected facial masculinizing plans, forehead surgery may instead focus on increasing brow prominence or adding forehead height.
Some surgeons "offer" gender-affirming surgery among many other services. For Align, it's the center of our life's work. Our 6 board-certified surgeons have committed themselves wholly to gender-affirming care and research. This means we aren't just helping patients reach their embodiment goals; we are helping to make their care better—better understood, better supported, and performed in the best way possible. What Is Recovery Like After Forehead Contouring?
Recovery after forehead contouring depends on the extent of the surgery and whether it is performed on its own or with other facial procedures. Most patients should expect swelling, bruising, numbness or tightness in the forehead or scalp, and temporary activity restrictions during the early healing period.
We will walk you through practical recovery planning during your consultation, including what to expect early on, when follow-up care is typically needed, and how recovery may differ based on the scope of your treatment plan.
How Much Does Forehead Contouring Cost in San Francisco and Los Angeles?
Gender-affirming care, including forehead contouring, is often covered by insurance. If you are using insurance, we will guide you through the authorization process and what to expect.
If you prefer to pay out of pocket, we will give you a personalized quote based on your surgical plan, including whether your forehead contouring is being performed on its own or with other related procedures.
Travel planning can involve a lot of moving parts, especially when you're coordinating around surgery. If you're coming to Align from outside the Bay Area or Los Angeles, we have resources specifically for you.
Risks and Considerations
As with any surgery, forehead contouring carries risks, such as bleeding, infection, scarring, swelling, numbness, healing concerns, and contour irregularities. Recovery can also involve prolonged swelling or changes in sensation while the area heals.
It is also important to approach forehead contouring with clear expectations. The best outcomes come from careful planning, clear priorities, and a realistic understanding of what surgery can and cannot do.
Forehead Contouring FAQ
Can forehead contouring be combined with hairline advancement?
Yes. In some cases, forehead contouring is combined with hairline advancement when both the forehead shape and hairline position are part of the surgical plan. This can also affect incision placement and surgical access.
How do surgeons decide how much contouring is appropriate?
Your anatomy and goals determine how much contouring is appropriate, including the brow bone and frontal sinus structure, as well as the kind of upper-face change you are hoping to achieve. Your surgeon uses these factors to determine the safest and most appropriate approach.
What procedures are commonly combined with forehead contouring?
Forehead contouring may be combined with procedures such as hairline advancement, frontal sinus setback, or orbital contouring. The right combination depends on your anatomy and the changes being planned.
