Facial feminization surgery (FFS) is a set of gender-affirming face procedures that can reshape the facial features to create more feminizing facial cues. For many patients, FFS is about bringing different facial features into alignment so the face feels more consistent with how they want to be seen. Rather than following one standard formula, FFS is tailored to the specific facial features, proportions, and contours you want to address.

At Align Surgical Associates in San Francisco and Los Angeles, California, facial feminizing surgery may involve one focused procedure or a broader surgical plan. Our role is to help you understand your options clearly, look at how different features relate to one another, and recommend an approach that fits your anatomy and priorities.

What Is Facial Feminizing Surgery?

Facial feminizing surgery refers to a group of face procedures that can change the contour, proportion, and emphasis across different areas of the face. Depending on the procedures involved, FFS may be performed in one operation or staged over time.

Who Might Consider Facial Feminizing Surgery?

Facial feminizing surgery may be a meaningful option for transfeminine individuals, nonbinary patients, and other gender-diverse people seeking feminizing facial changes. You may be looking for a more comprehensive plan or exploring whether one or two focused procedures would better fit your embodiment goals. Many patients consider the procedure to

Facial Feminizing Surgery by Facial Region

Facial feminizing surgery can involve one or more areas of the face, each with its own role in contour, projection, and facial cues. Prior facial surgery, fillers, or other treatments can shape what options are available and how your treatment is best structured. We’ll guide you through how these areas relate to one another so your plan reflects what matters most to you.

Upper Face:
Forehead, Brow, and Hairline

The upper face is often a key part of FFS planning because forehead contour, brow position, and hairline shape can strongly influence facial framing.

Procedures in this region may include hair restoration, hairline advancement, forehead contouring, frontal sinus setback, brow lift, or orbital contouring. Depending on the plan, the goal may be to reduce bony prominence, adjust the position or shape of the hairline, or change the contour around the brow and upper eye area.

Midface:
Nose, Cheeks, and Surrounding Contour

Midface planning often focuses on how the nose and surrounding facial volume relate to the rest of the face. Rhinoplasty may change nasal shape, size, or projection so it sits more naturally within the overall facial balance. Facial fat transfer may be used to add or redistribute volume in areas where a softer contour is desired.

Lower Face:
Jaw, Chin, and Lip Area

The lower face can strongly influence facial width, projection, and profile. Mandible contouring and jawline contouring may be used to change jaw width or angularity.

Genioplasty changes chin shape or projection, which can affect how the chin relates to the lips, jawline, and nose. A lip lift or lip augmentation may also be considered when lip shape, volume, or upper-lip show are important considerations in the surgical plan.

Neck and Tracheal Area

The neck and tracheal area can also be included in facial feminizing surgery, especially when profile changes are part of your embodiment goals. One common procedure in this area is a tracheal shave, which reduces the prominence of the Adam’s apple area.

In selected cases, neck liposuction or related contouring procedures may also be part of your plan if the goal is to change the contour beneath the jawline or through the upper neck.

What Is Recovery Like After Facial Feminizing Surgery?

Recovery after facial feminizing surgery depends on which procedures are included and whether they are performed together or staged over time. Most patients should expect swelling, bruising, activity restrictions, and a healing timeline that can vary depending on the areas treated and the extent of the plan.

We will walk you through practical recovery planning during your consultation, including what support you may need after surgery, when follow-up visits are likely to happen, and how recovery expectations can differ between a combined plan and a staged approach.

How Much Does Facial Feminization Surgery Cost in San Francisco and Los Angeles?

Gender affirming care, including facial feminizing surgery, is often covered by insurance. If you are using insurance, we will guide you through the authorization process and what to expect.

If you prefer to pay out of pocket, we will give you a personalized quote based on your surgical plan, including whether your facial feminizing surgery involves one procedure or a combination of procedures.

Risks and Considerations

As with any surgery, facial feminizing surgery carries risks, such as bleeding, infection, scarring, healing concerns, and changes in sensation. Combined procedures may also involve a longer or more complex recovery than a less extensive surgical approach.

It is also important to approach FFS with clear, realistic expectations. Surgical recommendations are based on what is appropriate for your anatomy and the changes you want to make, not on adding more procedures than are necessary to achieve your goals.

Facial Feminizing Surgery FAQ

Is facial feminizing surgery one surgery or multiple procedures?

Facial feminizing surgery can involve one procedure or a combination of procedures. The right plan depends on your anatomy, priorities, and whether a focused or more comprehensive approach makes the most sense.

Can facial feminizing surgery be staged over time?

Yes. FFS can be staged when that better fits your recovery preferences, priorities, or surgical planning. Some patients choose to address one region first before deciding on additional procedures.

Can I still be a candidate if I’ve had prior facial surgery or fillers?

Yes, in many cases. Prior surgery or fillers do not automatically rule out facial feminizing surgery, but they can affect planning, timing, and which options are most appropriate.
