Transgender or gender-affirming rhinoplasty is a facial procedure that reshapes the nose to create a more feminine or masculine appearance. Depending on the surgical plan, this procedure can change the bridge shape, tip shape, width, projection, nostril size, or overall nasal proportion. At Align Surgical Associates in San Francisco and Los Angeles, California, rhinoplasty is used to support feminizing, masculinizing, or more individualized embodiment goals.

What Is Rhinoplasty?
Rhinoplasty is a surgical procedure that changes the shape and structure of the nose. It may involve reshaping bone, cartilage, or both, depending on what aspects of the nose are being addressed.
In selected cases, rhinoplasty may also treat structural or breathing concerns alongside the aesthetic changes. We’ll go over what changes are possible for your unique anatomy and goals during your consultation.
-
We lead with your goals and make decisions—together.
Your plan starts with what matters most to you, such as function, sensation, privacy, recovery realities, and your day-to-day life.
-
We give honest recommendations.
We'll be transparent about what a technique can and can't reliably do for your anatomy and priorities, and we'll explain why we're recommending a particular approach.

We treat the whole person.
We don't reduce care to a single concern or body area. We look at your goals, your comfort, and what's safest and most supportive for you overall.

We prioritize privacy, dignity, and emotionally safe care.
We understand that many patients come in with complex past healthcare experiences and strive to create an environment where you always feel respected, welcomed, and seen at every step.

Who Might Consider Rhinoplasty?
Rhinoplasty may be a meaningful option for transgender individuals, nonbinary patients, and other gender-diverse people seeking changes in nasal shape, proportion, or definition. You might consider rhinoplasty if your nasal bridge, tip, projection, width, or overall size does not feel aligned with the rest of your facial features.

Feminizing vs. Masculinizing Rhinoplasty
Rhinoplasty can be customized to achieve both feminizing and masculinizing embodiment goals.
Feminizing Rhinoplasty
For feminizing goals, rhinoplasty often focuses on refining the nasal tip, narrowing the bridge, softening the overall nasal shape, or adjusting projection and rotation where appropriate.
Masculinizing Rhinoplasty
For masculinizing goals, rhinoplasty may focus on increasing straightness, projection, width, or structural definition. In some cases, the treatment plan may emphasize a stronger bridge or a more angular nasal shape.
Some surgeons "offer" gender-affirming surgery among many other services. For Align, it's the center of our life's work. Our 6 board-certified surgeons have committed themselves wholly to gender-affirming care and research. This means we aren't just helping patients reach their embodiment goals; we are helping to make their care better—better understood, better supported, and performed in the best way possible. Meet the specialists behind the transformations.
What Is Recovery Like After Rhinoplasty?
Recovery after rhinoplasty depends on the extent of the surgery and whether it is performed on its own or with other facial procedures. Most patients should expect swelling, bruising, and temporary activity restrictions, with final results developing over several months.
We’ll review what to expect during recovery and follow-up so you can feel supported and prepared at each stage of healing.
How Much Does Rhinoplasty Cost in San Francisco and Los Angeles?
Gender-affirming care, including rhinoplasty, is often covered by insurance. If you are using insurance, we will guide you through the authorization process and what to expect.
If you prefer to pay out of pocket, we will give you a personalized quote based on your surgical plan, including whether your rhinoplasty is being performed on its own or with other facial procedures.
Travel planning can involve a lot of moving parts, especially when you're coordinating around surgery. If you're coming to Align from outside the Bay Area or Los Angeles, we have resources specifically for you.
Risks and Considerations
As with any surgery, rhinoplasty carries risks, such as swelling, bruising, infection, healing concerns, asymmetry, and temporary or longer-lasting changes in sensation. When structural changes are part of your plan, your surgeon may also discuss breathing, nasal support, and internal healing.
It is also important to approach rhinoplasty with clear expectations. Even subtle changes to the nose can make a noticeable difference, and factors such as prior surgery and anatomical constraints can limit what is possible.

Feminizing rhinoplasty generally focuses on refining and softening nasal features, while masculinizing rhinoplasty may focus on increasing straightness, width, projection, or structural definition.
Can rhinoplasty address breathing or structural concerns, too?
Yes. In selected cases, rhinoplasty may address structural or breathing concerns in addition to the nose’s appearance.
