Monsplasty is a masculinizing gender-affirming surgery that reduces or reshapes the mons pubis, the tissue above the genitals. This procedure may be a fit for patients who want a flatter contour, less fullness in the area, or a surgical plan that better supports their broader embodiment goals.
At Align Surgical Associates in San Francisco and Los Angeles, California, we approach monsplasty with clear information, thoughtful planning, and recommendations grounded in your anatomy, priorities, and overall surgical plan.

What Is Monsplasty?
Monsplasty focuses on reducing or reshaping the mons pubis to create a flatter, smoother contour. Depending on your anatomy and goals, it may involve removing excess fat, skin, or both.
During monsplasty, your surgeon makes an incision just above the pubic area and adjusts the tissue to reduce bulk and improve the shape of the mons. The incision is then closed carefully to support healing and keep scarring as discreet as possible.

Who Might Consider Monsplasty?
Monsplasty may be a meaningful option for transmasculine, nonbinary, and other gender-diverse patients who want to reduce fullness in the mons pubis area, improve comfort in clothing, or create a more balanced contour. You might also consider monsplasty if excess tissue in the area affects how other gender-affirming procedures are planned or how their results appear.
Some surgeons "offer" gender-affirming surgery among many other services. For Align, it's the center of our life's work. Our 6 board-certified surgeons have committed themselves wholly to gender-affirming care and research. This means we aren't just helping patients reach their embodiment goals; we are helping to make their care better—better understood, better supported, and performed in the best way possible. How Much Does Monsplasty Cost in San Francisco and Los Angeles?
Gender-affirming care, including monsplasty surgery, is often covered by insurance. If you are using insurance, we will guide you through the authorization process and what to expect.
If you prefer to pay out of pocket, we will give you a personalized quote based on your surgical plan, including whether your monsplasty is performed on its own or combined with other procedures.
Travel planning can involve a lot of moving parts, especially when you're coordinating around surgery. If you're coming to Align from outside the Bay Area or Los Angeles, we have resources specifically for you.
Risks and Considerations
As with any surgical procedure, monsplasty carries risks, such as bleeding, infection, scarring, healing concerns, and changes in sensation. While the procedure is generally straightforward, it is still important to understand the risks and follow aftercare instructions closely.
It is also important to plan monsplasty as part of your overall goals. If it is being combined with other procedures, your surgeon will explain how that affects timing, healing, and the final shape of the area.
Monsplasty FAQ
Who may be a good candidate for monsplasty?
Monsplasty may be a good fit for patients who want less fullness in the mons area, improved comfort in clothing, or a surgical plan that supports their broader embodiment goals.
Can monsplasty be combined with other gender-affirming procedures?
Yes. In some cases, monsplasty is performed with bottom surgery procedures such as metoidioplasty, phalloplasty, or urethroplasty.
Will monsplasty leave a visible scar?
Monsplasty does involve a scar, as the procedure requires an incision, typically placed just above the pubic area. Your surgeon will place and close it carefully to support healing and keep scarring as discreet as possible.
