Gender-affirming breast augmentation, or chest feminization, is a form of top surgery that creates a fuller, more feminine chest contour. It may be performed with implants, fat transfer, or a combination of techniques, depending on your anatomy and desired result. At Align Surgical Associates in San Francisco and Los Angeles, breast augmentation is planned with close attention to your existing chest anatomy, available tissue, body proportions, and embodiment goals. Our role is to help you understand how these factors shape your options and recommend an approach that makes sense for your needs.

What Is Breast Augmentation?

In the context of gender-affirming surgery, breast augmentation is a chest surgery that creates or enhances the breasts using either implants or the patient’s own body fat. Your surgical plan is shaped by factors such as your existing breast volume, chest dimensions, and the contour that sits most naturally on your frame.

Our Commitment to You.
  • We lead with your goals and make decisions—together.

Your plan starts with what matters most to you, such as function, sensation, privacy, recovery realities, and your day-to-day life.

  • We give honest recommendations.

We'll be transparent about what a technique can and can't reliably do for your anatomy and priorities, and we'll explain why we're recommending a particular approach.

  • We treat the whole person.

We don't reduce care to a single concern or body area. We look at your goals, your comfort, and what's safest and most supportive for you overall.

  • We prioritize privacy, dignity, and emotionally safe care.

We understand that many patients come in with complex past healthcare experiences and strive to create an environment where you always feel respected, welcomed, and seen at every step.

  • We're built for gender-affirming care.

    Gender-affirming surgery isn't a side offering at Align—it's our life's work.

Who Might Consider Breast Augmentation?

Breast augmentation may be a meaningful option for transfeminine, nonbinary, and other gender-diverse patients seeking more fullness, projection, or definition in the chest.

Many patients explore chest feminizing surgery after hormone therapy has produced some breast development but not the amount of volume or shape they were hoping for. Others may be looking to refine their chest contour, symmetry, or overall proportion.

Hormone Therapy, Breast Development, and Surgical Planning

While hormone therapy can stimulate breast growth, the results can vary widely. This matters for your surgical planning because the amount of breast tissue you have affects the amount of implant coverage available, the degree of projection that can be achieved, and whether fat transfer may have a role in your surgical plan.

Our board-certified surgeons recommend allowing your breast development to progress before moving forward with breast augmentation. In general, waiting 12 to 24 months after starting estrogen gives your chest more time to change naturally so we can determine the best surgical technique for you.

Your Breast Augmentation Options

Breast augmentation can be performed with implants, fat transfer, or a combination of techniques. Some plans are designed to create more noticeable volume, while others focus on smaller adjustments in fullness, shape, or contour.

Breast Augmentation With Implants

Breast implants are the most common option for feminizing top surgery and can be customized based on your goals for volume, projection, and upper-pole fullness. We offer both saline and silicone implants and will guide you through discussions on implant type, implant placement, and incision options during your consultation.

Breast Augmentation With Fat Transfer

Fat transfer breast augmentation uses liposuction to collect fat from another area of the body, such as the thighs or abdomen. This fat is then purified and injected into the breasts to add fullness or enhance the chest contour. While it offers a modest size increase on its own, it may also be used to camouflage visible implant edges and add soft, natural coverage to implants.

Some surgeons "offer" gender-affirming surgery among many other services. For Align, it's the center of our life's work. Our 6 board-certified surgeons have committed themselves wholly to gender-affirming care and research. This means we aren't just helping patients reach their embodiment goals; we are helping to make their care better—better understood, better supported, and performed in the best way possible. What Is Recovery Like After Breast Augmentation?

Recovery after breast augmentation depends on whether your surgical plan includes implants, fat transfer, or both. Most patients should expect soreness, swelling, activity restrictions, and temporary changes in chest sensation during early healing. If fat transfer is part of your plan, you will also have a second area (the donor site) to manage.

We will walk you through practical recovery planning during your consultation, including what support you may need after surgery, when follow-up visits are likely to happen, and what to expect as swelling settles and your final shape develops.

Gender-affirming care, including breast augmentation, is often covered by insurance. If you are using insurance, we will guide you through the authorization process and what to expect.

If you prefer to pay out of pocket, we will give you a personalized quote based on your surgical plan, including whether your breast augmentation involves implants, fat transfer, or a combination of techniques.

Risks and Considerations

As with any surgery, breast augmentation carries risks, such as bleeding, infection, scarring, asymmetry, healing concerns, and changes in sensation. Implant-based procedures may also involve risks such as implant malposition, visible rippling, rupture or deflation, capsular contracture, and the need for future revision surgery to replace or remove the implants.

It is important to approach feminizing chest surgery with clear expectations. Your results are shaped not only by implant size and surgical technique, but also by factors such as tissue volume, chest dimensions, skin elasticity, and the extent of change your anatomy can safely support.

Breast Augmentation FAQ

How is chest feminization different from cosmetic breast augmentation?

Chest feminization is a gender-affirming form of breast augmentation. While it uses many of the same surgical techniques as cosmetic breast augmentation, planning often involves additional considerations such as hormone-related breast development and each patient’s unique embodiment goals.

Do I need to be on estrogen before breast augmentation?

Not always, but hormone therapy can affect your breast growth and impact the timing and planning for your surgery. In many cases, waiting 12 to 24 months after starting estrogen gives the chest more time to change naturally before moving forward with breast augmentation.

What incision placement options are available for breast augmentation?

Incisions are often placed in areas where the resulting scars can be well hidden. Options include the crease beneath the breast, called the inframammary fold, or around the edge of the areola.

Where can breast implants be placed?

Breast implants can be placed either beneath the chest muscle or above it. We recommend submuscular placement for patients with limited breast tissue, as it can provide softer coverage and reduce visible rippling. However, placement above the chest muscle may be appropriate for certain patients.
